Welcome to Kindergarten! Logansport Community School Corporation is
excited to announce their new KINDERGARTEN REGISTRATION process.
Effective this year, ALL incoming kindergarten students will register at our
corporation-wide registration event which is being held on April 27th at
Landis Elementary School. This is an exciting time in your child's life, and
your child will begin a most exciting time in their lives in the fall: attending
school.
When you come to Berry Beginnings Kindergarten Registration on
April 27th, please bring proof of the following:
1. IMMUNIZATIONS. Students must have the following immunizations
in order to enroll in school:
• 5 DTap
• 4 Polio
• 3 Hepatitis B
• 2 MMR
• 2 Varicella
• 2 Hepatitis A
2. LEGAL BIRTH CERTIFICATE
3. PROOF OF ADDRESS
4. SOCIAL SECURITY CARD (if available)
Also, it is strongly recommended that your child have a complete PHYSICAL and
DENTAL exam (see Kindergarten Physical Exam and Immunization Record
Sheet).

¡Bienvenidos a kínder! La Corporación Escolar de la Comunidad de
Logansport se complace en anunciar el nuevo proceso de INSCRIPCION
PARA KINDER. A partir de este año, TODOS los estudiantes que vayan a
ingresar a kínder se inscribirán en nuestro evento de inscripción a nivel
corporación que se llevará a cabo el 27 de abril en La Escuela Primaria
Landis. Este es un momento emocionante en la vida de su hijo/a, y su hijo/
a comenzará el momento más emocionante de su vida en el otoño: asistir
escuela.
Cuando vengas a las inscripciones para Berry Beginnings y Kínder
el 27 de abril, por favor traiga comprobantes de lo siguiente:
1. VACUNAS. Los estudiantes deben tener las siguientes vacunas para
poder inscribirse en la escuela:
• 5 DTap (tétanos, difteria, tos ferina)
• 4 poliomielitis
• 3 hepatitis B
• 2 MMR (sarampión, paperas y rubéola)
• 2 varicela
• 2 hepatitis A
2. ACTA DE NACIMIENTO LEGAL
3. COMPROBANTE DE DOMICILIO
4. TARJETA DE SEGURO SOCIAL (si cuenta con una)
Además, se recomienda altamente que su hijo/a tenga un examen FÍSICO y
DENTAL (referirse a la hoja de Registro de Vacunas y Examen Físico para
kínder).
